Dysfunctional Thought Record

Hundreds of ‘automatic’ thoughts pop into our minds every day, often without us noticing. Depending on their content, these thoughts can lead us to feel different emotions, like joy, anxiety, anger, or sadness (to name a few). Our thoughts can also be biased: not every thought we have is 100% accurate or helpful.

This Dysfunctional Thought Record will help you monitor and re-evaluate your negative automatic thoughts by exploring alternative perspectives.

Complete this record whenever you notice a negative thought or strong change in your mood. Each entry should take you between 5 and 10 minutes.

Overview

The Dysfunctional Thought Record is a worksheet to record and challenge dysfunctional thoughts. It encourages clients to identify cognitive biases (or "unhelpful thinking styles") which are operating ad develop more balanced ways of thinking.
