Health Anxiety Thought Record

Hundreds of ‘automatic’ thoughts pop into our minds every day, often without us noticing. Depending on their content, these thoughts can lead us to feel all sorts of emotions – joy, excitement, fear, anxiety, or sadness. Our thoughts can also be biased: not every thought we have is 100% accurate or helpful.

The Health Anxiety Thought Record can help you:

  • Become more aware of your automatic thoughts about your health.
  • Re-evaluate and respond to thoughts which leave you feeling anxious.

Complete this diary whenever you become anxious about your health or as your therapist has instructed you. Each entry should take you between 5 and 10 minutes.
