Intrusion Record

Intrusions (also known as intrusive thoughts) are distressing thoughts or images that pop into your mind unintentionally. This Intrusion Record will help you gather important information about your intrusive thoughts, including what they are, the meanings you attach to them, and how you try to cope with them.

Complete this record whenever you notice an intrusive thought, or as directed by your therapist.

Overview

What sets intrusive thoughts in obsessive compulsive disorder (OCD) apart is the significance that individuals with OCD attach to them. The Intrusion Record is a cognitive behavioral therapy (CBT) worksheet designed to capture the occurrence and meaning of intrusive thoughts and images.
