Intrusive Memory Record

Intrusive memories are thoughts about past experiences that pop into your mind without you choosing to think about them. They can feel very vivid and upsetting, and may come with strong emotions and physical sensations, as if the event is happening again. Intrusive memories are often triggered by things around you, such as sights, sounds, or smells.

The Intrusive Memory Record is a tool that will help you gather information about your intrusive memories. This should help you start to understand these experiences better and the role they play in difficulties such as post-traumatic stress disorder (PTSD).

Complete this record each time you notice an intrusive memory, or as directed by your therapist.

Overview

The Intrusive Memory Record helps therapists and clients document the content, frequency, triggers, and emotional intensity of intrusive memories. These involuntary recollections differ from deliberate memory retrieval and are often vivid, distressing, and sensory in nature. Logging these memories over time can support clinical decision-making, guide targeted interventions, and track therapeutic progress in trauma-focused treatment.
