Panic Attack Record

Panic is a surge of intense fear, usually accompanied by strong bodily sensations and catastrophic thoughts. When these feelings come on very suddenly, it is called a panic attack. Panic can feel terrifying, but it isn’t dangerous.

This Panic Attack Record helps you record details of your panic attacks, including any ‘near misses’ (times when you felt very anxious but avoided having a full panic attack). This information will help you understand your panic, guide your treatment, and track your progress. 

Complete this record whenever you experience a panic attack or near miss, or as your therapist has instructed you.
