Urges – Self-Monitoring Record

Urges and cravings are strong desires to have something or to do something, like drinking alcohol, smoking or vaping, taking drugs, or gambling. Urges can feel overwhelming at times and contribute to serious problems, including addictions.

Cognitive behavior therapy (CBT) suggests that the way we think and feel influences these urges. Keeping a self-monitoring record can help you become more aware of what triggers your urges, your responses to them, and the role your thoughts and feelings play.

Complete this record whenever you experience an urge or as you therapist has instructed.

Overview

The Urges – Self-Monitoring Record worksheet is designed to help clients capture information about their urges and cravings. It includes columns to record information about: situational context; the focus and intensity of urges; cognitive, emotional, and physiological reactions accompanying urges; responses to the urges; and consequences of those actions.
