Thought Challenging Record – 7 Column

The Thought Record is an essential tool in cognitive behavioral therapy (CBT). Thought challenging records help people to evaluate their negative automatic thoughts for accuracy and bias. This 7-column version encourages reflection uopon the evidence for and against a negative thought.

The thought challenging record is a fundamental tool in cognitive behavioral therapy (CBT). The underlying principle can be summarised as “what do you believe, and why do you believe it?”. This seven-column thought record can be used to:

  • Identify automatic thoughts
  • Help clients understand the links between thoughts and emotions
  • Examine the evidence for and against a selected automatic thought
  • Challenge an automatic thought
  • Generate more realistic alternatives to an automatic thought
  1. Start by cueing the client’s memory for the automatic thought by directing them to think about where & when the thought occurred. Record this in the first column.
  2. The cue for completing a thought record is usually a sudden change in emotion. In the second column record the emotion felt and it’s subjective intensity (0–100%).
  3. In the third column record the automatic thought. Helpful prompts are “what were you thinking about when you started to feel that way?” or “what was going through your mind as you started to feel that way?”. Automatic thoughts can be images as well as thoughts. In the case of an image ask the client to reflect on what the image meant (e.g. if the client has an image of themselves frozen to the spot it may have idiosyncratic meanings ranging such as “I’m weak”).
  4. If there are multiple thoughts then select one to work on.
  5. In the fourth column record the evidence for the selected thought.
  6. In the fifth column record any evidence which contradicts the thought. Clients may need help to overcome their ‘blind spots’. Depersonalise the issue by asking what a friend might say.
  7. In the sixth column write a new thought which synthesises all of the recorded information. The new thought may be considerably longer than the original thoughts. It may not necessarily be positive, the aim is to counter bias in the original thought. Rate the degree of belief in the new thought.
  8. In the final column re-rate the client’s emotional state in light of the updated thought, rate the intensity of the emotion now.

In therapy clients often need assistance and practice at identifying the link between thoughts and emotions before they move on to challenging thoughts and substituting more helpful thoughts for less helpful ones. Some clients might find it helpful to practice identifying automatic thoughts using a Simple Thought Monitoring Record before introducing the complexity of evidence-gathering and thought challenging.
